End-stage renal disease (ESRD) in patients with aHUS due to factor H mutations are a challenge because there is very high risk of disease relapse in an isolated kidney graft, requiring preventive high dose plasma therapy or eculizimab treatment. Successful liver-kidney transplantations have been performed in several patients with heterozygout factor H mutations. We report the first patient with a homozygote factor H mutation and successful liver-kidney transplantation.Immunosuppressive regimen was basiliximab induction, oral prednisone, tacrolimus and mycophenolate mofetil. Three months post transplant serum creatinine was 75 mmol/l, proteinuria is negative and immunosuppression is well tolerated. All hepatic markers are normal.
